Zinc oxide cream is a simple, widely available, and effective way to protect irritated perianal skin. It creates a barrier between the sensitive skin and the things that irritate it — moisture, friction, and contact with stool. If you are dealing with raw, red, or irritated skin around the anus, zinc oxide is a practical first step.
How it works
Zinc oxide is a physical barrier. When applied to the skin, it forms a protective layer that:
- Blocks moisture from sitting against the skin — reducing maceration (skin breakdown from prolonged wetness)
- Reduces friction between skin surfaces
- Protects from irritants — including enzymes in stool that can damage skin
- Provides mild anti-inflammatory effect — helping to calm redness and irritation
- Creates a healing environment — damaged skin heals better when protected from ongoing irritation
When to use it
Zinc oxide is helpful for:
- Perianal irritation from frequent bowel movements or diarrhoea
- Pruritus ani (anal itching) — protecting the area from moisture and friction that perpetuate the itch-scratch cycle
- Post-surgical drainage — protecting the skin around a healing wound from ongoing moisture
- Fistula discharge — the area around a draining fistula can become irritated from constant moisture
- Incontinence-related skin damage — even minor leakage can cause significant skin irritation over time
How to apply
- Clean the area gently — after a bowel movement or sitz bath
- Pat completely dry — zinc oxide works best on dry skin
- Apply a thin layer to the external perianal skin — you do not need a thick coat
- Reapply after bowel movements and cleaning
- Do not insert internally — zinc oxide is for external skin only
Practical tips
- Apply after your prescribed treatment. If you use GTN, diltiazem, or another prescribed topical, apply that first. Allow it to absorb (five to ten minutes), then apply zinc oxide as a barrier layer.
- Use a generous amount if the skin is very raw — a thicker layer provides more protection.
- It will stain. Zinc oxide is white and can stain underwear. Use a pad or liner if this concerns you.
- Clean gently. Zinc oxide does not need to be fully removed between applications. A fresh layer on top is fine. Harsh scrubbing to remove it defeats the purpose.
- Choose unscented. Avoid products with added fragrances — the simpler the better for sensitive skin.

A typical perianal skin care routine where zinc oxide plays a role:
- Sitz bath or gentle water cleaning
- Pat dry thoroughly
- Apply prescribed topical treatment if applicable
- Wait five to ten minutes
- Apply zinc oxide barrier cream
- Pad or liner if needed for protection
